Ask Dr. Kaiser Question: Here I am again, up all night, in chronic pain and extremely tired but I cannot sleep. I was in a car accident a year ago, head on at 60 miles per hour, and I broke nine of my back right ribs. About a month passed after the accident, before I started noticing pain in my hips and legs, which is is now throughout my entire body, even my skin. I am on enough oxycontin to knock out a ox, but I remain awake, and extremely tired. I live in Spokane, WA and I cannot seem to find a doctor here to help me. I am in so much pain but I am also tired all the time. I can no longer run my little shop and I also now have short term memory loss. Please help if you can.
Dr. Kaiser replies: It sounds like, since your unfortunate accident a year ago, that your nervous system has become severely depleted from both the stress of the event plus the ongoing drain of energy that your chronic pain can cause. This nervous system depletion can be responsible for severe fatigue, insomnia, and a perpetuation of your chronic pain cycle. There is no way out of this unhealthy cycle without putting together a comprehensive healing program that includes appropriate vitamin supplementation to strengthen your nervous system, hormone balancing to support your body and give you more energy, gentle exercise such as stretching or yoga, and low doses of medications that help the nervous system become stronger. This would include both narcotic and non-narcotic pain medications, as well as low doses of a carefully chosen antidepressant, not specifically to treat depression, but to help your nervous system get stronger in the face of these enormous stressors.
