Question:

I stopped taking all of my meds (Zerit, Viracept, Epivir)about one month ago due to lipodystrophy. Two weeks after that I developed a fever that has continued ranging from 99 to 102.5. My doctor thinks it could be because I stopped taking any meds, some kind of rebound effect. No cause has been found from the blood work. Shall I just keep eating Cipro (an antibiotic) in the hopes that the fever will subside? Thank you.

 

Dr. Kaiser replies:

In my experience, HIV infection that is not being treated with antiviral medication only causes fever if the viral load is unusually high, at least 75,000 copies/ml or greater. You don't mention in your question whether this is the case nor what level your CD4 count is currently at. If your CD4 count is below 200 cells/mm3 you are at risk for an opportunistic infection and should be worked up as such. At the very least, blood cultures, a CBC, a chest x-ray, and a sinus series x-ray should be done to rule out common sources of infection.
